Faith Behavioral is a home providing shelter and support services for behavioral health residents who have been deemed not to be a danger to others or themselves. The objective is to assist them in furthering their treatment while providing them a safe place to live and where their basic health (behavioral) and medications can be monitored.

The service population is adults, age 18 and over with mental illness.

Counseling will be provided for each resident at the frequency and duration prescribed in his/her treatment plan. Counseling may occur in a group setting or individually as stated in the treatment plan. Individual counseling may not exceed one hour every two weeks and group counseling will not exceed one hour every month. Counseling topics will be consistent with the program goals and treatment plans.
o Life Skills: Personal goals, stress management, health issues
o Education: GED, vocational and/or college options
o Personal Development: Values, behavior management, social skills, anger control, relationships, communication skills
o Therapeutic recreation
o Crisis management
This agency will not provide family counseling.
Qualified personnel will assist residents on an as-needed basis with self-administration of medications. Assisting with self-administration will include one or more of the following:
1. Storage of participant’s medication
2. Reminders of medication times
3. Opening a medication container for a participant
Assisting with self-administration will include Verifications:
I. Medication is being taken by the participant for whom it was prescribed
II. Dosage taken matches that directed on the container’s label
III. Confirming that resident took medication as prescribed
IV. The BHT will observe the participant taking the medication to prevent errors/overdose
FBHG will provide transportation assistance for residents to and from appointments per the resident’s treatment plan.
1. Consistent with each resident’s treatment plan, FBHG will organize off-site activities such as shopping, recreation or education.
2. As necessary, FBHG will provide assistance with self-administration of medication while resident is off-site.
Criteria for admission and readmission are the same.
1. Persons age 18 and over with a qualifying SMI diagnosis
2. Medical concerns do not necessitate 24 hour nursing supervision
3. Demonstrate behaviors consistent with DSM V mood disorders, anxiety disorders or thought disorders which require and can reasonably respond to therapeutic interventions.
4. The resident can maintain a less restrictive environment to stabilize his/her condition.
5. The resident has identified case management needs such as assistance with application and maintenance of medical and/or financial benefits, linkage to other agencies and providers, advocacy services, clinical outreach and/or community-based interventions, ongoing need for clinical monitoring and supervision services and intensive support treatment.
6. Resident presents issues that can be effectively addressed through FBHG Program services inclusive of Life Skills, Educational, Personal Development, Assistance with Self-administration of Medication, and Recreational Programming.
7. Have mental health medication and diagnosis that does not exceed the supervision capabilities of these scope of services.
8. Must be a willing participant in the collective treatment and be willing to abide by applicable house rules
9. Must not be currently a physical or severe psychological threat to themselves, to personnel, or to other residents
10. Must not have repeatedly damaged property or have recent documented cases of theft of agency or resident property
11. At this time hearing or visually impaired residents cannot be accommodated at FBHG
12. FBHG can provide services for non-English speaking residents by contract with a language services interpreter.
Should a resident require additional services or have special needs which cannot be provided, FBHG, in conjunction with The Placement Agency (e.g. Mercy Maricopa Integrated Care, Navajo Nations, NARBHA etc.) will make a referral to a more appropriate setting.
1. Referred services may include specialized assessment, assistance, assistance with care and treatment planning, outpatient services, or specialized counseling.
2. If family counseling is part of a resident’s treatment plan, it will be provided by referral for off-site services by another agency as specified in the plan
3. Medical and dental services will be provided through local hospitals or clinics as needed
4. Personnel will contact local authorities on issues of medical attention that need emergency assistance by calling 911.
5. Psychiatric emergency services will be provided by local crisis centers in conjunction with the placement agency.
6. Residents will be referred through local community college districts for educational assessments and employment needs.
A resident will be discharged from FBHG when:
1. The resident’s treatment plan goals have been substantially met and a discharge plan has been discussed and agreed upon by the clinical team, resident and/or resident legal representative;
2. The resident chooses not to participate in the treatment program, i.e., the resident withdraws consent for treatment;
3. The resident no longer meets admission criteria;
4. A probation, parole, or criminal violation requires;
5.The resident is assessed to have therapeutic needs that exceed the scope of services provided by the facility;
6.Consultations with case manager and placement agency conclude that the resident may be a danger to others or himself;
Involuntary discharge may result from:
I. being absent without authorization;
o engaging in unprovoked physical abuse of another resident or personnel;
II. attempting suicide,
III. arson, gang involvement, continued drug/alcohol use,
IV. consistent failure to attend work or school,
V. or a documented pattern of noncompliance with house rules and or the program.
All transferring of residents for direct services at a different healthcare institution with the intent that the resident will not return to FBHG will be accommodated by the placing agency/RBHA. FBHG will provide the necessary documentation, summaries, and history as part of any permanent transfer to another behavioral health agency initiated by the placing agency source.
The minimum resident to personnel ratio: 5:1 / 24 hours per day or higher as required for residents with issues requiring intense supervision.
Residents who can afford to pay a portion of their share of cost will be requested to pay an amount based on a sliding scale of 30% of their income each month. This amount will be in writing in their residency agreement and will be disbursed by a trust, payee or fiduciary when available.
Any overpayments will be refunded to payment sources within fifteen working days from date of notification or realization in a form of a check.

Administration hours are Monday through Friday, 8am to 5 pm and as needed on weekends. The administrator or administrator’s designee will be available on-call after hours and on weekends and holidays.
FBHG provides behavioral health services 24 hours per day, seven days per week. Behavioral Health Technicians (BHT) and /or Behavioral Health Paraprofessionals (BHPP) will be scheduled 24 hours per day, seven days per week.
